I’m 46 and an otolaryngologist in Türkiye. I’ve been experiencing pain in my right knee when flexing it deeply, but I initially thought it was a minor ligament injury from working out that just needed some time to heal. I used to do squats and other leg exercises without any problems.

Two weeks ago, I felt a sudden pain in the same knee while playing volleyball. The MRI result was: *Horizontal cleavage tear of the posterior horn of the medial meniscus, extending to the meniscal apex and dissecting the meniscus.*

An orthopedic friend of mine told me that surgery is necessary because I’m an active sportsman (although not a professional), and that this type of tear is almost impossible to heal with exercise or any other non-surgical treatment. Do you agree with him?

I really don’t want surgery, mainly because I don’t want part of my meniscus to be trimmed and my knee to become weaker as a result. I’d really appreciate your opinion. Thanks in advance!

I'm 72 in great shape and no issues ever with my knees going to the gym for decades 3-4x a week, walking/hiking daily and doing yoga in my 50s and 60s. 5 months ago my medial right Meniscus root tore. MRI showed 1. Medial meniscal posterior root radial tear with 4 mm of meniscal
extrusion. Grade II chondromalacia of the medial compartment.
2. Subchondral fractures of the medial femoral condyle and medial tibial
plateau with moderate and mild marrow edema respectively.
3. Large joint effusion with synovitis.
I stayed 13 weeks non weight bearing to let the bone heal, weeks 14 and 15 adding 10 lbs a week weight while on crutches and walker.
Everytime I asked about the meniuscus the Orthopedic Dr said "don't worry about that now' 4 vists over 4 months and last visit he said try walking around the house without crutches" and "I don't need to see you anymore" I said "what about the fixing the meniscus?? He said "Your 72 and it's not an option at this point" What?!
