    The emphasis on lowering LDL earlier feels especially important. As a Doctor of Pharmacy who carries APOE4/4, I appreciate the focus on cumulative exposure and the overlap between cardiovascular and brain health.

    I consider my self a lucky man as Dr. Michael Davidson is my lipid doc. I heard his name on another Attia cast and mentioned he was at U of Chicago. I live in Chicago and sought him out. I think I have the best!

    The real argument buried in this episode isn't about LDL, it's about timing. Most cardiovascular and cognitive decline gets treated as a problem to manage at 60 when the decisions that mattered were made at 30.

    Dr Davidson said a couple of times that once you are diagnosed with MCI it is too late but please look at the Bredescen research and Dr. Perlmutter's new book that challenges that assumption.  Please expound on this

    Brain-Specific Role of APOE and HDL<br>In the central nervous system (CNS), cholesterol metabolism is entirely separated from the rest of the body because circulating lipoproteins (like LDL and HDL) cannot cross the blood-brain barrier (BBB). <br><br>The brain manufactures 100% of its own cholesterol locally.<br><br>APOE in the Brain: <br>Apolipoprotein E is the primary lipid-transport vehicle in the brain. It is produced mostly by astrocytes to shuttle cholesterol and phospholipids to neurons for membrane repair, synaptic plasticity, and myelin maintenance. <br><br>Your APOE 3,3 genotype is the neutral, baseline standard; it does not carry the structural liabilities or increased Alzheimer&apos;s risks associated with the APOE 4 variant.<br><br>Brain-Specific HDL: <br>In the brain, APOE molecules combine with local lipids to form small, discoidal, HDL-like particles. <br>There is no &quot;LDL&quot; network in the brain parenchyma; lipid clearance and recycling rely entirely on these nascent brain HDL particles.<br><br>The Homocysteine Threat:<br>Even with a favorable APOE 3,3 profile, high homocysteine can directly impair APOE3 function. Research shows that elevated homocysteine interferes with the dimerization (the binding together) of APOE3 proteins. This restriction reduces their ability to generate healthy brain HDL particles, effectively depressing your neutral APOE3 efficiency down to a level that mimics an APOE4 risk state.

    ApoE 2/2 is rarely mentionend. &nbsp;High longevity but also high triglycerides. &nbsp;Poor LPL function. &nbsp; And TGs are a risk! &nbsp; And you see, high HDL is important.
