  It has always seemed to me that at the moment a taxiing airplane reports that they&apos;re unsure where they are the controllers should stop every plane cold where it is and halt all takeoffs and landings until they are 100% sure that all planes have been accurately located and any potential conflicts resolved.<br><br>Allowing a lost plane to continue to wander around is ridiculous.

  Northwest crash… I think it’s unfortunate and possibly unfair that the captain lost his job, but the FO lying about his military service and experience is such a statement about his character that I’m glad he was fired.

  Re: Detroit accident. Aswell as stopping the aircraft and reporting to the Tower that they were unsure of their position and requesting a lead vehicle, the crew would have done well to file an Air Safety Report to their Safety Department reporting the inadequate state of the taxiway markings and signage to be forwarded to the FAA as a mandatory occurrence &nbsp;ln turn, an inspection by the FAA would have confirmed the crews&apos; concern and then ordered an immediate rectification. Unfortunately the accident occurred before these inadequacies were identified. Very surprised that Northwest had not already implemented CRM training programme by the time this accident occurred. So avoidable in so many ways but the classic example all of the holes lining up on the Swiss cheese model.

  It&apos;s just so painful listening to everyone just persisting in their errors. No-no we won&apos;t admit we made a mistake! We will go ahead right until someone dies. Ugh

  First story: &nbsp;An officer who retired after 20 years as a Major had not impressed his superiors with his abilities. &nbsp;Most US military officers retire as Lieutenant Colonels or Colonels after 20 years (Commanders or Captains in the US Navy). &nbsp;Promotion to Major is pretty much automatic and is achieved at the seven or eight year point in a career. &nbsp;So this first officer spent 12 or 13 years at the same rank, viewed by his superiors as a mediocre officer during that time.

  I used to work security at YYC Calgary Airport and once in awhile we get those blizzards that you can barely see through, especially at night. I lived out of town and work started at 4:30 am. <br><br>I remember thinking about what the pilots were seeing as they came in because my windshield on the highway was literally icing up from the inside, even with the defroster and heat at full blast. I was able to follow trucks into the city though. &nbsp;<br><br>If you take 1 tbs of dawn dish soap and maybe 6 tbs or more of distilled vinegar and mix them together, you&apos;ll get a neat chemical reaction that does remind me of how the blizzard looked at night on my windshield.

  &apos;Bring out your favourite drink, find a comfortable place to sit and relax&apos;. I think that giving this advice to pilots would greatly increase Mentor Pilot&apos;s output!

  I just noticed that the Tenerife tragedy would make it in most of these long hauls. The Air Transat incident made me realize that its quite common for pilots to want to climb or descend in such emergencies when the helpful thing to do would be to level off for a while then you can pitch down or up with clarity of where you&apos;re going. When speed is the issue, there are only so many things you can do to deal with it, one of them being the thrust. I understand that its an unexpected situation but pilots should instinctively check the thrust setting when they&apos;re too fast or too slow. Here we have both ends of the spectrum and somehow, the thrust setting is the last thing the pilots consider
