  The ValuJet incident just makes me so mad. So preventable. The FAA continues its head in the sand attitudes about mandating things that could help prevent accidents and incidents. The NTSB does a great job ferreting out issues that create problems but sadly, they do not have regulatory authority. The FAA so often gets too cozy with the entities they are supposed to regulate and either makes the recommendations optional or softens the requirements so they are more like suggestions.

  Valujet Story: &nbsp;I&apos;m a retired US Navy Chief Petty Officer. &nbsp;Fire is considered the worst casualty on any ship. &nbsp;Flooding or collision damage is easier to contain than fire, which requires the emergency workers to be wearing oxygen masks or air breathing apparatus and to deal with heat.

  Lufthansa reasoning is totally absurd, it assumes that a pilot can assess mental and physiological conditions, just by looking or listening to the other crew member! The German Wings case shows, it’s almost impossible for un untrained eye, to detect signs of deep mental distress. This case shows the same for physiological conditions, which are even unknown to the patient! Why did EASA thought that airlines knew better about safety, procedures than they did? Isn’t it the institutional task of these agencies to provide unbiased expertise on safety matters?

  I&apos;ve always suspected more people have died due to the secure cockpit rule, than would have been threatened without it. This situation wouldn&apos;t have happened if the rule hadn&apos;t been instigated.<br>It&apos;s like the paradox of home security - you protect your house against burglars, but that makes it difficult to escape in case of fire.

  There was another Vauljet accident. Vauljet flight 597, they didn&apos;t even make it off the ground without an engine fire that destroyed the plane. No one was kill on the plane thank goodness.

  I used to have seizures and in my case I would generally lose complete consciousness for almost 20 minutes followed by a period of severe confusion. The people on that Lufthansa flight were fortunate that the FO regained some useful consciousness after such a short time and the Captain was able to regain control.

  The Germanwings event was a consequence of what is termed as an &apos;inadvertent transfer of risk&apos; following the events of 9/11 and the changes to cockpit entry procedures.
