I find vaginal DHEA (prasterone) particularly interesting because the vagina is not exclusively an estrogen-responsive tissue. Vaginal cells also have androgen receptors and contain the enzymes needed to convert DHEA locally into several active sex steroids, including testosterone, DHT, estrone and estradiol.<br><br>That is essentially the idea behind vaginal DHEA: instead of directly administering an active hormone, you provide a precursor that can be converted locally within the vaginal tissue. This is known as an “intracrine” mechanism. The tissue can locally produce both androgenic and estrogenic hormones and then metabolize them again.<br><br>This raises an interesting question: why not simply use a vaginal estradiol + testosterone gel?<br><br>In principle, that is pharmacologically possible, and vaginal testosterone has actually been studied. However, there are some potential advantages to using DHEA as a precursor. With estradiol + testosterone, you are directly administering two biologically active hormones and have to determine the appropriate dose of each. Both hormones can also potentially be absorbed systemically, and testosterone can subsequently be converted into DHT and estradiol.<br><br>With DHEA, the approach is different: you provide the tissue with a relatively weak precursor and allow the vaginal cells themselves to produce the steroids they need locally. In other words, instead of directly supplying two active hormones, you are using the tissue’s own steroid-producing machinery.<br><br>Clinical studies of vaginal DHEA have shown improvements in vaginal dryness, dyspareunia, vaginal pH and the vaginal epithelium, while serum hormone levels generally remain within the postmenopausal range. Longer-term studies have also not shown evidence of endometrial stimulation.<br><br>However, I would not interpret this as meaning that vaginal DHEA is superior to vaginal estradiol. Low-dose vaginal estradiol is extremely well studied and is highly effective for genitourinary symptoms of menopause. DHEA simply works through a different mechanism: estradiol provides estrogen directly, whereas DHEA can serve as a local precursor for both estrogenic and androgenic activity.<br><br>The really interesting point is that the vagina appears to respond to both estrogenic and androgenic signaling. DHEA is an elegant way of potentially supporting both pathways locally without directly administering systemic doses of testosterone and estradiol.<br><br>That said, the evidence base for vaginal estradiol is still considerably larger. So I would not say that DHEA is “better.” It is more accurate to describe it as an alternative approach that makes physiological sense because it allows the vaginal tissue to generate multiple sex steroids locally.

General practitioners and nurse practitioners do not sell supplements in Canada nor do they have the types of testing available in office that Dr. Haver says she has in hers. In Canada a GP or NP has to refer/prescribe you these types of tests.<br>It would be nice if these experts recognized that these issues exist in other countries other than the United States.

Dhea was 100 times better for me than estrogen cream—it helps with orgasms which Estrogen cream did nothing.
