The reason he was mistaken was because when the bell hop called out George Kaplans' name, Cary Grant raised his hand and the two hit men assumed that Grant was Kaplan.

In the hotel he called over the telegram boy when they were paging for George Kaplan. That is how they mistook him for Kaplan

Thornhill was mistaken for George Kaplan because when he waved to the boy to ask to send a telegram to his mother the boy was paging Kaplan so when Van Dam's henchmen saw that they assumed Thornhill was responding to the page.

Back in the day people quietly paid attention to conversations and beginnings of movies. People didn't skip over until action or intensity happened. So reactors always miss the beginning of the film, where you see how they mistook him to be Kaplan.

At the restaurant, Cary Grant happened to raise his hand to call for the bellboy so he could send his telegram to his mother, at the same time, another bell boy was walking past calling out for Mr Caplan. (Another decoy type action arranged by the spy group to create the impression that Mr. Caplan was staying there.) The 2 gunmen happened to see this from across the room and assumed that Grant was Caplan.

I learned of some interesting facts about James Mason recently. He was friends with Sam Neill. He saw Sam's performance in My Brilliant Career and reached out to him. He was so impressed he bought Neill a plane ticket to visit him in Switzerland. James introduced him to agents etc. and took him under his wing as kind of mentor.
