  I USED TO HAVE THIS GAME!!! I remember being to scared to even play it in broad day light during college cause the atmosphere with headphones was too much for me lol

  I think it's possible it is implied you are in fact a dead body. The nurses, the wheelchairs, those 'ticks' could be organs. You have a grim reaper style/executioner style guy with a big weapon and a bag over his head and then you have the bugs which naturally are attracted to dead bodies. The guys crawling on the ceiling's weakness being their 'tail' could simply be the same as you in a sense, souls trying to escape and by pulling them down you end their escape. It could be your soul is metaphorically escaping into a better place by the end of the game. Further emboldened by the ending skull guys, being well possibly escaped dead people themselves leading you to an afterlife. The act of confinement and wanting desperately to escape, being the idea of your body stuck in a coffin and feeling enclosed could also be a part of this.
