I believed he was actually healed from it but instead of living with the fact he killed his wife and knowing his kids died he decided to pretend being insane again so he could be lobotomized so he could forget vs living with it. So I believed he finally broke through but couldnt live with it and decided to "die" thinking he was a good guy vs someone who killed his wife.

He didn't regressed, he just made a conscious choice. His last line is EXTREMELY important.

The best thing about this movie is that when it starts, we're under the same delusions as Leo, and when we learn the truth, we feel the same mental and emotional breakdown as he does. It's brilliant take on unreliable narration.

I love how this is a completely different movie the second time you watch it.

He faked slipping back into delusion because he couldn’t live knowing he killed his wife because his wife killed their children. He wanted to forget.

One of my favourite things about this film is that it's an actual place that was trying to help people, instead of a place where they torture or experiment on patients

I love the use of fire & water to help distinguish what is illusion vs reality. That's why the woman's water glass disappeared. He only chose to see it when it was empty, because he's avoiding reality. This is such a fun rewatch to catch little details, like the staff being so on edge around him, his partner fumbling with his gun, or his clothes not fitting because it's just a costume.

This is a movie that will require a second viewing. On the second watch, you already know what happens, so you end up catching a ton of little things you never spotted before because you didn't know to look for them.

Check out "Mystic River" it's a movie adaptation of a book by the same author that wrote "Shutter Island"
