A lesser reported one, Neil Peart. He died of the same brain cancer Gord Downie had... he was playing through issues towards the end. No one (public) knew he had cancer until he was gone. RIP Neil.

Not rock, but he was a jazz drummer, Buddy Rich had a lot of heart problems, but played until he died. At the Concert for the Americas in 1982, where he played what has been called the impossible drum solo, he started having a heart attack in the middle of the drum solo. In the video footage of the concert, you can see when he first gets that stab of pain, then continues on playing the best drum solo in history.

I would add Michael Nesmith to this list. Though in ill health, he insisted on performing the final tour with Micky Dolenz as the Mike and Micky show. He passed away one month after his last appearance with Micky.

So sad how in life you are living life the way you want but can’t get a chance to sit back and soak it in and think about how great your life was.

There could be a two hour video on this with Gospels Singers. Especially Black Quartet Singers. They been know to die while singing on stage.
My Son played drums for a Band named 9day Trip. He managed to give great performances despite suffering from Reno Disease. He would do dialysis on three days a week. Saturdays after dialysis , I pick me up . He get home, shower, Then borrow my van to haul his drum kit to a gigs mile away. I'd offer to drive him ,But he say "I got it Pop".
I guess all of us Musicians tend to push our bodies beyond any illness. I nearly passed out playing the Organ at a funeral. Only to go to the emergency room after ,Thinking I had bad allergies . Only to find I had heart failure.

Arthur Kane, bassist for the New York Dolls, played the Metldown for Morrissey in England and then flew back to Salt Lake City. He went to see his doctor and died three hours later.

Tragically Hip's final tour with Gord Downie was so emotional. The entire tour was a huge news story in Canada, and their final concert was televised live on National TV, with almost 1/3 of the entire country watching the performance. Every country or region has a performer/band that they have a special bond and identify with, and Tragically Hip with Gord as the front man were that band for Canada.

How is Ronnie James Dio not on this list? Dude was still killing it onstage while dying of stomach cancer.
