In Fallout 4 it is implied that the pack raiders from nuka world are cannibals since you can find "strange jerky" in their base

There's a guy in Far Harbor, at Echo Lake Lumber Mill, who openly admits to being a cannibal. He even carries a unique shredder.

I think a good honorable mention to all of this human hunger is how in fallout 4, there’s the Longneck cannery. That place is run by a dude who put ghoul meat into the food and causes a bunch of people to get sick

I love how much they leaned into the cannibalism idea in 76. Even though it fell short in many ways, 76 had some really awesome ideas

When you ask Bob for some Iguana Bits after you confront him, he says something like that: "You what that they are made of and you still want one?! You make Decker look like a kid" . That pretty much confirms, that Bob is not himself a cannibal.

I think we should mention hillarious way of roleplaying as aspiring cannibal in Fallout: New Vegas to get the perk "Meat of Champions". There's something special in eating King, Caesar, Kimball, House (and probably all other leaders if you feel like it) to show everyone that you're the apex predator of Mojave.

As someone from Appalachia (specifically Western PA if you care lol) there are legends of folk in the mountains who lost their mind and went cannibalistic, some people believe they are responsible for folks that go missing along the Appalachian trail, and then also the Algonquin legend of the Windigo
