  Similar to the Witcher, Demon's Souls & Dark Souls have armour which looks practical while having a lot of character. FromSoft's designs became more fantastical as the games went on, but those early games look like something you'd realistically wear.

  There's actually a mod for KCD2 that makes the armor even more authentic, because some of the real nerds just weren't content with the original.

  Somebody made witcher style armor pieces with chainmail directly put on leather "plates". And it performed fairly well, with good flexibility and good protection against attacks you can expect from monsters.

  The thing with geralts armour having buckles in the front, is that he's normally in the wilderness, he needs to be able to take it off and put it back on himself, even in that video you showed someone else was doing up that knights buckles for them, he's also needs those belts to hold his alchemy potions

  Interesting fact about the flair in the back of Roman helmets... it's designed to protect the neck. A person's natural response when a strike is heading towards them it to flinch. When the soldier flinched, they plate in the back moved forward to protect the neck from a strike.

  As a side note, I really love how in Kingdom Come: Deliverance helmets with visors actually limit your vision while you have your visor down. It adds a lot to immersion, but also adds a very real debuff that makes you not want to have your visor down all the time.
