My conjecture but this is the Soviet Union. The firefighter probably died of 'chemical exposure' or 'trauma relating to a fall'

Hospital records being incomplete makes 100% sense given that it was an overwhelming influx of patients and everything was filed on paper. He could have arrived as an emergency case where paperwork would be figured out after, and in the chaos, it was never filed.
