Happened upon this creator, good production skills, The only thing is the panel inside is a sub panel and the green bond screw shouldn't be installed.

The bonding screw is supposed to be in the panel outside by the meter

Good catching the problem. And this is exactly why all splices and taps must be made inside of an approved enclosure. Too often particularly in older homes where work has been done by unqualified personnel, you might find open splices in attics, basements, crawlspaces and sometimes even buried inside walls and ceilings. I've seen it and that's a tragedy waiting to happen.

Always torque down the connections to the proper specs. Loose connections cause high resistance and heat.

Most likely the guy installing the generator and adding the load shed modules failed to tighten those taps I would think.

I’ve used these taps (we refer to the as Clear taps) for years. Always pretty reliable. Definitely bad connection will cause this.
