Even though a smile is meant to show happiness , a lot of times it's creepier and scarier than a blank or angry expression

This movie is a metaphor about dealing with trauma and having noone to turn to. She was vulnerable to the smile demon because she still havent dealt with her past trauma..thats why she kept dreaming about her mom, she blames herself..and that is why she became a therapist, to help others since she couldn't help her mom. As you see through out, not one person actually took the time to listen to her(except her ex boyfriend, they cop)...and this movie basically shows that when someone has problems, listen to them, take them seriously, support them..no support can lead to more trauma and for some, suicide.

16:36 Seems like you noticed it, but when the camera panned to Carl, he was fast asleep. A subtle hint that her interaction with Carl was completely a hallucination in her head.

Smile is inspired by a short film called Laura Hasn't Slept who starred Laura the PhD student who reprised her role for this film.
