An Interesting But Unfortunate Medical Case

A senior of mine has said that a smoker of long time that stopped smoking is a red flag, even if they refer no symptoms they probably stopped smoking 'cause they weren't feeling too well... As an intern I found it useful to never miss it, especially because we don't have radiologists comment x-rays in Portugal

I think that this is a clear reminder to everyone to get your regular STD panel. Monogamy or otherwise, it's so important to test just in case you need to treat.

Great video. Very informative. It's really nice to see your thought process and decision making as the case progressed. I think the most confusing thing about this patient is how sick he was (based on his electrolytes, liver enzymes, cd4, white count, infections) vs the reason he came into the hospital. He was dealing with symptoms of severe illness for about a month but it sounded like if he hadn't been losing sleep from the hiccups and if his girlfriend hadn't been around, he may not have come in at all. Your team did a lot for him and tried to give him a chance, but it really sounds like the opportunity to save him was missed when he presented two weeks prior to the outside hospital with headache. Not blaming them of course because who knows how that interaction went or if he articulated all of the symptoms he had been having, but if anyone would have had a chance to save him, it would have probably been then. Thanks for making this video. I really hope it gets a lot of views so you can make more like it and help us all become better doctors.

Wow, this was such a well thought out and organized presentation of a very interesting case with a heartbreaking ending. I am sorry to hear of both this and the outpatient passing so close to one another.<br>Your patients and all future learners are incredibly lucky to have such a great physician teacher as yourself though! I can&apos;t thank you enough for these videos you put out, they are fantastic

Thank you. Learning a lot. With sudden death following ambulation, massive P.E. comes to mind.

So so sorry for the losses... [hug] (inevitable in choosing this career though!) But really appreciate case presentations of this kind, (please do more, perhaps happier ones next time lol!) - so informative and educational (esp. for MS4 / interns) - having gone through the case with all these up and downs, and having really learned a lot. Thanks❤

Thank you for making this video. It was very thoughtful and thorough. It&apos;s really helpful to see the evolution and thought process, especially with a poor outcome. Your honesty with the outcome and your reflections on it was very valuable.

Amazing video! I love how you walk us through your thought process and tie in relevant lectures within the case presentation. Wish I found these sooner definitely going back to watch all the others. Please keep them coming!
