Can you make provisions to change beneficiaries, etc in WA State or is this just for NY?

My husband passed away in a nursing home while on medicaid, so in Ohio estate recovery kicks in when I pass. Can I still set up a mapt to protect my home if I should be institutionalized or will I still owe for his care because it wasn’t in place when he was receiving benefits?
