Though during the epilogue you do get a shred of hope for her future if you took the route of her going to Avernus when she tells you about the blueprints they found (that is if you send Wyll with her and not gone yourself). Don't wanna put too much detail in here of course cuz of spoilers.

If she romances Gale he can ascend her to godhood and thus remove her engine. But only in an origin Karlach run.

Good news, everyone; Karlach lives!
She's in Heroes of Faerûn and Adventures in Faerûn set years after BG3. So, as far as 'canon' goes, she's vibing back in Faerûn. 🎉

Yes her story is very sad.

But Karlach is the very most important person. The most human. We each have a ticking time bomb, in the game ONLY Karlach has that, and her bravery shines through as she shows how we too can accept that end, with head held high.

She has repeatedely said that she would rather die than live in Avernus. Instead burning up sitting at the end of the dock, is the true end. For those of us who understand or even suffer terminal illness, this makes her our true hero!

Personally, I believe EA Wyll is the most tragic. Getting erased due to incessant whining from players is pretty tragic.
