  In Paper Mario: The Thousand-Year Door, a ghost asks you to retrieve his diary, but not read it. If you try to read it, you get many warnings to discourage you, but once you read the first page, the ghost ends you.

  Nier Automata Ending K: you eat a mackerel that the game CLEARLY tells you it will kill you because you’re an android and you shouldn’t be eating human food.<br><br>But you do and it kills you. At least the death scene says it tasted good.

  &quot;Cooking&quot; a grenade for too long.<br><br>A surprisingly large number of games.<br><br>To clarify this is the act of pulling the pin on the grenade but not throwing it right away to reduse the time your opponents have after you throw it. Or in this case holding it until it explodes.

  I like that even though getting trapped in the vault in New Vegas was silly game ending mistake it still got a proper voice over ending sequence, I’d like to see every silly death in that game get the same treatment. “And so the courier was killed by some scrub bug out in the wasteland, ..they forgot to pause when they we’re checking their phone.. and isn’t that just a tortured metaphor for the true nature of man..”

  Speaking of nitroglycerin, you can pick some up in Hitman. If, for some reason, you decide you no longer want to carry it, DO NOT DROP IT. It explodes. Immediately. Yes, I figured that out the hard. Yes, I REEEEALLLY should have known. No, this is not a throwaway account for Mike.

  I&apos;m surprised you guys missed dating Morinth in Mass Effect 2. You know, the Ardat-Yakshi that refused to stop melding with people despite the fact that it... kills them. The one who&apos;s mother you killed to recruit her? &quot;Don&apos;t worry Shepard, you&apos;re not like other humans, I&apos;m confident you&apos;ll survive it!&quot; Yeah... you had that game over coming.

  In &quot;Let It Die&quot; there is an item called the Gambleshroom. When eaten, this shroom has a chance to either fully heal you, or turn you into a frog (temporarily). If you happen to use one of these and get the latter effect while a lizard is coincidentally nearby, it can eat you.
