For low risk patients, the number needed to treat (NNT) for statins to prevent a heart attack is over 200, yet the number of patients who will have a side effect from taking statins is about 1%. These side effects can include muscle pain, fatigue, headaches, constipation, diarrhea, sleep problems, and potentially increased blood sugar level levels.

Mr. Feldman was hugely influential in my time on keto, and I'll freely admit that I maybe leaned too hard into the idea of LMHR phenotype offering protection against the atherogenic effects of LDL. If I remember correctly, there was some analysis of the lipid triad (Framingham?) he presented that, when stratified for high HDL and low TG, effectively nullified the effects of LDL on ASCVD.<br><br>Given the vertical incision down my chest, though, I became complacent in that knowledge and waited too long to determine that I was in pretty bad shape. I blame only myself for failing to push harder for a CAC early into my keto journey, and am unable to point any fingers at any particular phenomenon or idea. If I were to address Dave's followers, I'd urge them to better quantify their health condition (not just risk via lipid panels, but actual harm via the CAC) as well as to closely monitor it.

It’s interesting to me as a clinician too , and a person with heart disease, there is a huge mistrust of medical clinicians , and guess what ,,a lot of this is because patients do there work too , and learn , and ask questions about metabolic health , and I would say most physicians have no idea what this really means , they have no idea about the pathways that can help heal cardiovascular disease , AmpK1 , autophagy Fasting , ketosis ,, so yes most of my colleagues don’t have any idea about this , so it’s not surprising to me , especially with the advent and use of AI for medical education and people are sick and tired of being sick and tired , they are tired of being reactive instead of proactive , ie functional medicine is on the rise , people will pay out of pocket to get healed , not just another management tool … <br>We need to get to the root cause instead of just managing symptoms .. Period .. If a physician doesn’t start learning , they will be a dinosaur and outdated with integrative health or functional medicine ..

It can be both rather than either. Statins reduce the risk of heart attacks and strokes with 100+ studies and 40+ years backing that assertion. Also, some supplements may also help. Nattokinase for example with reasonable evidence supporting it. We need to stop attacking the other side and rather find the best of both sides of the treatment paradigm.

Lily starts from a pro-drug stance, so I take her opinions with a grain of salt but when you look at statins from a perspective of absolute risk reduction the benefits almost completely evaporate. Even in patient populations with existing CVD the benefits do not look that impressive. And benefits in this population are more than likely pleiotropic effects in reducing underlying inflammation.
