Since the EFHW feedpoint impedance is very high, the return current is relatively small. In many installations the capacitance of the coax shield to ground is sufficient to provide that return path. The downside is that the coax then becomes part of the antenna system, so moving or rerouting it can change the antenna's impedance, SWR, and radiation pattern.
