The reservoir had never been full — the water was still 3.3 ft below the spillway when the dam broke. Eleven people died, and that number is the luckiest thing in the story. Two federal panels investigated. They did not agree. On the grout curtain — the one thing the whole design depended on — the Independent Panel found the rock "not adequately sealed," with leakage "capable of initiating piping." The Interior Review Group concluded seepage through that same curtain was "within tolerable limits," and the curtain "satisfactory." Same dam, same evidence, twelve months apart. This video explains what they disagreed about, and then the answer engineers give today: that both panels were arguing about the wrong thing. Reclamation's current position is that the decisive fact was filter incompatibility, which made a defect-free core "virtually impossible." Every dimension in the 3D reconstruction is read off the signed 1971 construction drawing (DWG 549-D-8), reproduced in the Independent Panel report.
