    The lieutenant seems awesome.  He clearly cares for his patients and his crews…the most important ingredient in the making of an exceptional medical professional.  Our system worked differently, we weren’t integrated with a fire service, our fire service was almost all volley.  Our EMS was paid.  We had six stations.  Two all BLS and the rest combined.  Each BLS only station had a First Lieutenant as Station Chief.  All ALS and ILS units had a 2nd lieutenant as unit commander, which was the limit of rank for an ILS provider to encourage them to get their full ALS training.  Their pay would rise with years but most did go for their paramedic.  Paramedics with experience got their first lieutenant fairly quickly and got first due and were ALS crew chiefs at each station.  I got my Captain and was shift supervisor before I got my BSN in emergency nursing and my trauma nurse.  Most Captains had their FP-C or CC-EMTP.  Station Chefs were Majors or Lt. Colonels.  Specialists like field nurses or Swat Medics were Commanders which was below Colonel but above Lt. Colonels.  They were only called for unique situations but like staff officers.  We had three Colonels who helped the Chiefs who were the old guys.  Interesting times.  You guys seem like a good company and supportive.
