  Despite the various mistakes and errors made by all parties leading up to this catastrophe, the most mind boggling thing for me is that an airport that was surrounded by 20.000ft obstacles was even allowed to operate without radar installations

  My father was a pilot, and he told me more than once: &quot;Always trust your instruments. &nbsp;Your brain will lie to you, and you will think things are happening, which are not. &nbsp;The instruments are dumb, they don&apos;t know HOW to lie, so always trust them.&quot;<br><br>The only time he ever contradicted that viewpoint was, &quot;if you have 2 different sources of information, and you are getting two completely different answers, then one of them must be wrong. &nbsp;At that point, your only chance is if your gut is right.&quot; &nbsp;Which is why on a lot of modern systems, you have 3 or more indications of things, so that you can detect an instrument error. &nbsp;But an instrument is more likely to be right than the human perception, &nbsp;especially when under stress.

  The investigator that died was my Grandfather&apos;s brother, Gordon Corps. The Royal Aeronautical Society still hold an annual talk in his honour.

  RIP Simon Carlo Petrie. Carlo was on the return leg of his gap year and was just 22 years old. His loss was devastating for his mum, dad and sister as well as his friends. It is hard to believe its 33 years ago but we visit his memorial at Nottingham Trent University whenever a reunion takes us back.

  Having lived and worked in Thailand for nearly 12 years now, I think a little cultural context for &apos;Westerners&apos; might be helpful here. Being a pilot for Thai is a super high status job. Meaning that the talent pool is vanishing small - and yes, as the stepfather of a UK based A320/1 pilot I&apos;m only too aware of the costs of entering the profession, even in relatively egalitarian/meritocratic Europe. Pilots in Thailand are thus drawn almost exclusively from the upper middle class/upper class, either directly through family connections or via the military (which is itself seeped in nepotism/cronyism). This means that these guys (and a few gals) are equipped with a mind set that is very difficult to explain to Westerners, seemingly deferential but in reality almost completely impervious to outside influence, and I speak as someone who used to teach senior officers in the Thai military, where I saw this tendency in spades. Hyper self-confidence and hyper self doubt exist in tandem, and when pressure is applied to such a personality, paralysis is going to be the least of your problems. My guess is that this cultural baggage, along with the class/age dynamics between this pair were the direct cause of this catastrophe.

  Remember, 59 days after Thai Airways International Flight 311 (July 31, 1992), on September 28, 1992, Pakistan International Airlines Flight 268 ALSO crashed in the Himalayas, near Kathmandu. That was triggered by a sort of &quot;off-by-one&quot; error, where the crew unknowingly entered the altitude for the following Distance Measuring Equipment marker instead of their current one due to mishandling their approach chart. For example, by 16 miles, they should have been at 11,500 ft, but they descended to 10,500, where they should be by 13 miles. Bizzarely, they reported being at 11,500. By 13 miles it was 9,500&apos; instead of 10,500&apos;, then 8,250&apos; instead of 9,500&apos; by 10, etc, but they DID report their ACTUAL altitudes there. That accident claimed another 167 lives, the deadliest in Nepalese history. Only after THAT accident was radar actually added at Kathmandu, just to let you know.
