I think the "go outside and have a birthday" line indicates the recruiter did want to enlist the protagonist. Essentially telling him to walk out and come back in lying about his age, hence "have a birthday". Also explains the other lines during the finale as a flashback from the protagonist's pov

The "go out and have a birthday" was a common tongue in cheek joke telling young men to lie about their age. Essentially youre too young so just go "get older" and then youre legally old enough to fight

I don't think anyone has mentioned this before, but the Mother boss could allude to the nurses who took care of soldiers during world war 1. They were often referred to as "mom" by the soldiers, since they were a comforting female presence taking care of the soldiers and nursing them back to health... and, as we know, many soldiers were children lying about their ages.

It's not like you take off the mask and are instantly all better. You still see some monsters, but you never see Whales, Impalers, or Puppets. Just Pale Soldiers, just things that look WAY more human than monstrous. Even the game script calls them "shapes", like you're not seeing them exactly, just vague shapes since your face and one eye have been destroyed, and the other possibly heavily damaged.<br><br>The gas makes you see everyone as a monster. That's why the Flamer calls you a monster, he's still hooked up to the gas, he still has his mask on, and he's still being puppeted by the higher-ups in this war.

There's also a thing called Trench Mouth. It's caused by the same conditions as Trench Foot. Basically war is hell, no one wins, except the people selling the shovels

The helmet being referred to as an "enemy" helmet could also be implying things about the character's psychological state. He might be unable or unwilling to distinguish the dead by side, either because he believes he's alone to the point where he considers them all enemies (as he's trying to desert he might very well have to fight through his own nation's troops), or because he just doesn't want to admit he looted the corpse of an ally (wartime traditions on looting are an odd thing, taking from the dead of the enemy is seen as more acceptable than taking from your own).
