Given all the miserable bosses I ignored, I can probably milk this topic quite a bit more. What bosses do you think belong on this list?

I would argue that Lady Maria is the one who suffers the most in Bloodborne. While Ludwig did indeed suffer watching his legacy crumble before his eyes, he's not exactly cognizant of what has transpired. When we meet him, he's a beast through and through. Only after we finish the fight, does Ludwig understand how much of a fool he was made to be by the church. Even then, the hunter is able to lie to spare him the gory details of how everything he's worked for has gone to shit.

Unlike many of the other bosses of Bloodborne, Lady Maria is fully aware of her sins and failings and is constantly surrounded and reminded of her failures in life. Right before her fight, you climb up the research hall which is filled with her experiments all of whom are asking her to save them from their fate. Knowing that she can't help them, she shuts herself in the Astral Clocktower trying to sleep away all of her pain. The boss fight before her, the Living Failures, indicates that even despite all the suffering she inflicted she was never able to replicate the old ones and ascend to godhood. When you first see her, she's lying in a chair covered in congealed blood bleeding from neck, and it's heavily implied that she took her own life rather than live with the consequences of her decisions. AND YET, she still has to live through those decisions for eternity because of the Hunter's Dream. Unlike the beastly Laurence or Ludwig, she's fully aware she's in hell and there to be punished.

When the hunter finally finds her atop the clocktower, she's well aware of the cycle of resurrection that the hunter is capable of, so unlike most other souls-like fight, she understands that she can never win against an immortal foe. Despite this she goes all out just to give the player "an honest death" i.e. to tilt the hunter so they do not make the same mistakes she did. When the fight doesn't go her way, she resorts to the one technique that she never wanted to use, her vile blood heritage. She had taken great pride in not having to use vile blood to achieve her level of prowess, ability, and status. However, in order to prevent you the hunter from making the same mistake she did and going to the fishing village, she humiliates herself and violates her sense of self-worth and identity just to defeat you. AND YET, she still fails and in her final moments has to live with the knowledge that she failed one last time.

Another obvious addition to this is Artorias and Sif, really Gwen's four knights in general

I can just feel the frustration of fighting Radahn and whiffing those charged R2s. I used the star fists for an entire playthrough but I had to switch because of Radahn’s weird hurt box.

Surprised you didn't mention ceaseless discharge - that guy has to have one of the most tragic stories

An interesting thing about Ludwig is that we don’t know if he ever turned into a beast in the waking world. He could’ve died human only to be forced into his beastly form in the nightmare without any chance of stopping it

Midra is my 1st thought. "Enough, I have endured more than enough.." just that line alone says a lot.
And him turning into a lord of frenzied flame sorta means he suffered as much as one Can possibly suffer(at least for a human). Otherwise the flame would not choose him.

From what I recall, Malenia was fully aware of Miquellas intentions with Radahn, based on what she whispered in his ear
